Abstract
Healthcare organizations in Gulf Cooperation Council (GCC) countries face growing challenges in quality of care, patient safety, and nursing workforce retention, with annual turnover rates ranging between 18.7% and 22.5%. This study examines the role of strategic transformational leadership in enhancing organizational transparency and employee empowerment, and their combined effect on institutional performance outcomes within GCC healthcare organizations.
A descriptive, analytical, and critical approach was adopted through a systematic review of 23 peer-reviewed studies and institutional reports retrieved from PubMed, Scopus, and Web of Science, applying predefined inclusion and exclusion criteria for literature published between 2010 and 2025, following PRISMA guidelines.
Transformational leadership — encompassing idealized influence, inspirational motivation, intellectual stimulation, and individualized consideration — significantly promotes organizational transparency across informational, procedural, and interactional dimensions, while elevating psychological and structural empowerment. These factors collectively improve quality of care, patient safety, job satisfaction, and workforce retention. Eight critical research gaps were identified, most notably the scarcity of multi-context GCC studies and the uncritical adoption of unadapted Western frameworks. The study proposes an integrated conceptual framework and a four-phase implementation model grounded in five complementary theoretical perspectives. Culturally adapted leadership models are recommended, alongside future empirical studies to validate the proposed framework within GCC healthcare settings.
